Gas and other measuring instruments [1]
The phenomenon of cross sensitivity in gas meters should be made more widely known.
Navigation
A BRM team has a verifying task and should identify and correct individual errors made by team members. All members of the BRM Team must therefore be aware of their responsibilities and job descriptions within the team. This means that if one or more members of the BRM team leave the bridge for a short or long period of time, the master (or another team member) will ensure that their tasks are fulfilled or taken over.
Bridge orders/alerting the master [2]
Ship's officers must be made aware of their duty to keep a better lookout and to be aware of what is happening around them - even from a distance - so that it is noticeable if a vessel (such as the Trueborn without lighting) is behaving suspiciously.

Use of electronic and other navigation aids [1]
Knowledge of radars/AIS/ECDIS and their integration should be improved. AIS remains visible on radars that are on standby.

Sailing under pilotage [1]
The pilot ladder should be made ready and checked by a competent officer well before the pilot disembarks. This preparation is even more important when using manropes.
“2.2. The rigging of the pilot transfer arrangements and the embarkation of a pilot shall be supervised by a responsible officer having means of communication with the navigation bridge and who shall also arrange for the escort of the pilot by a safe route to and from the navigation bridge. Personnel engaged in rigging and operating any mechanical equipment shall be instructed in the safe procedures to be adopted and the equipment shall be tested prior to use." - SOLAS CH V, Reg 23
Carrying out the work
When purging, a toolbox meeting must always be held in advance with the entire crew, during which the checklists are discussed: who does what and how. This ensures that the checklists are checked and, if necessary, supplemented.
A shipping company must give clear instructions for purging.
